POSITION SUMMARY

The Health Information Management Specialist is responsible for the creation, maintenance, and protection of patient medical records. This role requires strong attention to detail, excellent organizational skills, and the ability to communicate effectively with patients, providers, and staff. You will serve as a key point of contact for record availability and release of information while ensuring compliance with all applicable regulations.

ESSENTIAL FUNCTIONS

• Collect and maintain patient demographic, personal, and financial information
• Create, process, and maintain patient medical records within electronic medical record (EMR) systems
• Ensure timely availability of patient charts and medical records for healthcare providers
• Retrieve, track, and process medical records in accordance with chart-out procedures
• Process Release of Information (ROI) requests in compliance with federal, state, and local regulations
• Protect patient confidentiality and uphold HIPAA standards at all times
• Respond promptly to inquiries or appropriately escalate questions and issues
• Maintain accuracy and quality by following clinic policies, procedures, and documentation standards
• Communicate effectively with patients and staff in person and over the telephone
• Support clinic operations by identifying and reporting process improvement opportunities
• Perform other duties as assigned

WORKING ENVIRONMENT 

This role is based in a fast-paced, patient-focused urology clinic where collaboration is key. Our team of physicians, nurses, and support staff work together in a modern facility equipped with an EHR system and on-site diagnostic services. The atmosphere is professional yet warm, with a shared commitment to delivering compassionate, high-quality care to a diverse patient population.  The work environment characteristics described here are representative of those an employee encounters while performing the essential functions of this job. The noise level in the work environment is usually mild.  Potential exposure to infectious diseases, bloodborne pathogens and chemical cleaning agents. Electronic medical records (EMR) systems, phone systems and standard medical equipment.
